Enfermedad de Creutzfeldt-Jakob (Nueva variante). Mal de las vacas locas

Una variante atípica (no clásica) de Creutzfeldt-Jakob, hipotéticamente trasmitida por bovinos afectados de BSE al humano, fue registrada por primera vez en 1996 en el Reino Unido. Hasta hoy se han reportado 95 casos de esta nueva variante en el Reino Unido, un caso en Irlanda del Norte y tres en Francia. Presenta diferencias con la forma clásica esporádica, de rango de edad (14 a 53 años, media 28 años) aunque recientemente se ha reportado en un hombre de 74 años, clínicas -caracterizadas por un período inicial prolongado que puede ser de alrededor de seis meses con síntomas psiquiátricos y/o sensitivos antes de desarrollar el cuadro clínico característico similar al CJ clásico-; de resonancia magnética -por el que presenta una lesión hiperintensa en T2 en pulvinar a diferencia de la forma clásica que puede llegar a presentar esa señal en ganglios de la base-; neuropatológicas -presenta depósito en PrP en el sistema nervioso en forma de "placas floridas", no característico de la forma clásica-, y bioquímicas en el tejido nervioso (Western Blot), dado que presenta un pattern similar a BSE con mayor representación de las formas glicosiladas que de la banda no glicosilada. Quienes adquirieron la variante atípica en el Reino Unido, con una incubación aproximada de diez años, presentaban una particularidad o susceptibilidad genética (MM en el codón 129 del cromosoma 20). O sea, modificaban para metionina y eran homozigotas en dicho codon en el que existe un polimorfismo normal en los eres humanos. No se sabe aún si los que dodifican para Valina (VV) o los heterozigotas expuestos puedan presentar un tiempo de incubación más prolongado.
