  • Aterosclerosis: Hipotesis de la homocisteina (Kilmer McCully)

    Las enfermedades cardiovasculares son la causa principal de muerte en los países occidentales. Hasta hace poco se pensaba que los únicos factores de riesgo eran: colesterol alto, hipertensión arterial, diabetes y los efectos del tabaco; pero estos, en sí, no justificaban la alta incidencia de estas enfermedades en nuestra sociedad.
    Por esta razón se comenzaron a investigar otros factores de riesgo, donde se pudieron clasificar a sujetos que manifestaran problemas cardiovasculares pero en los que se pudiera descartar los cuatro factores enumerados anteriormente. En los últimos años cada vez se ha puesto más interés en los niveles de Homocisteina en plasma como factor patogenético en las enfermedades cardiovasculares o tromboembólicas (infarto de miocardio, accidente cerebrovascular, enfermedad vascular periférica y trombosis).
    En los años 60, al profesor de la Universidad de Harvard, Kilmer S. McCully que en aquel entonces desempeñaba labores de investigaciones sobre enfermedades hereditarias en el Hospital General de Massachussets, se encontró que un grupo de niños, de origen irlandés, presentaban síntomas de una enfermedad rara que se caracterizaba por los niveles anormalmente altos de un aminoácido llamado Homocisteína. La enfermedad presentaba síntomas de retraso mental, dislocación del cristalino en los ojos o crecimiento acelerado. En estos casos el hígado se ve incapacitado para metabolizar el exceso de homocisteina de la circulación sanguínea. Normalmente esta sustancia dañina se metaboliza al volver a transformarse en metionina, sustancia de la cual procede, y en segundo término puede convertirse en otro aminoácido azufrado llamado cisteína, fundamental para el organismo pero que en exceso es eliminado a través de la orina. Al estudiar casos de homocisteinuria McCully encontró el de un niño irlandés de 9 años, que tenía un hermano con la misma enfermedad, por lo que se reafirmaba el origen genético del padecimiento. Las arterias del niño, fallecido por un ataque cardiaco, estaban afectadas por arteriosclerosis avanzada. La circulación había sido obstruida por ateromas y coágulos de sangre. Después de estudiar todos los casos documentados hasta el momento el doctor McCully planteó una hipótesis, muy innovadora para la época: el colesterol, calcio u otras sustancias depositados en las paredes arteriales eran un signo pero no la causa de la aterosclerosis. El origen de los depósitos, sin embargo, estaría en unos anormales niveles de homocisteina. Su intuición le llevo a descubrir que todos los fallecidos de homocisteinuria presentaban deficiencia tanto de piridoxina, vitamina B6, como de ácido fólico (vitamina B9) y cianocobalamina (vitamina B12).

    Las autoridades sanitarias calculan que en España 14,5 millones de personas tienen el colesterol elevado: la mitad de los españoles mayores de 18 años. Pero, aunque la mayoría de la gente sabe que existe el colesterol, y que no es bueno sobrepasar ciertos límites, muy poca sabe qué es realmente, qué significa “tenerlo alto” y cómo debe combatirse.
    “Hay un montón de confusión y controversia alrededor del colesterol”, explica el profesor de medicina de la Harvard Medical School Dariush Mozaffarin en un reportaje de The Washington Post. “Hay confusión incluso entre los científicos que lo estudian”. Algo que, al final, acaban pagando los pacientes, que están muy perdidos respecto a lo que signica “tener colesterol”, una expresión tan extendida como imprecisa. “La gente habla y escribe sobre el colesterol como una grasa que obstruye las arterias”, explica Mozaffarin. “Pero esa idea de que comes algo, se cuela en el flujo sanguíneo y obstruye tus arterias es, sencillamente, falsa. Nada remotamente parecido a eso ocurre”. En 2006, un importante estudio realizado con 50.00 mujeres mostró que una dieta baja en grasas no logra proteger contra los infartos o los ictus y este mismo año, una revisión de estudios codirigida por Mozaffarin ha mostrado que las recomendaciones para evitar el consumo de grasas saturadas no ayudan a bajar los niveles de colesterol. Algo que refuerza la idea que vienen repitiendo numerosos científicos díscolos desde hace unos años: debemos dejar de culpar a la grasa de nuestra mala alimentación, y empezar a fijarnos en los carbohidratos y los azúcares. También en lo que respecta al colesterol. “Básicamente, los alimentos son algo más que el colesterol y la grasa que contienen”, explica Mozaffarian. “Necesitamos comer de forma saludable, comidas lo menos procesadas posibles (especialmente plantas, aceite de oliva y frutos secos) y no definir las comidas por los nutrientes que tienen o dejan de tener”. Sabemos que la dieta mediterránea, que es el régimen alimenticio que tradicionalmente hemos tenido en España, es mucho más saludable que una dieta baja en grasas, y sus bondades podrían residir, precisamente, en que apenas contiene comidas procesadas y carbohidratos, pero sí muchas frutas y verduras y pescado.

Es posible que sientas este dolor como inflamación, cansancio o debilidad en los músculos. El dolor puede ser una molestia leve o puede ser lo suficientemente intenso como para dificultar tus actividades diarias.

Por ello, se recomienda a los pacientes que han sufrido infarto de miocardio o accidente cerebral que mantengan cifras muy bajas de colesterol para intentar limpiar así sus arterias.
